А ты сам хачесс редачил или из архива заменял может с хостом чето не то
Скинь содержимое хачес

Попробуй это в хачесс вписать
Options +FollowSymlinks

как на 4ке сделать адрес анкеты в виде сайт/ид (site.ru/1)?
